but I use A 16F628A

maybe a bug!!
now i have to take pic out and USE with TL866 II PLUS

it's on a breadboard
yellow is on MCLR pin4 of cpu 16F628 conector 07
green is on PGD pin13 of cpu 16F628 conector 05
white is on PGC pin12 of cpu 16F628 conector 03
black red on plus and minus 5v conector GND 09 and plus 5v conector 01 with other cpu it works well pic 18f43k20 18f442
also on breadboard

Hi,
Try applying a pull-down resistor to PGM pin RB4. That may resolve the problem if the LVP bit is set in configuration bits (such as on a brand new device).
Disable LVP in configuration so that pull-down is not necessary next time you program the PIC
